8-9month with 103.4 fever help!

This is my first and I don’t have anyone to help me but she had a temp of 102 about four hours ago I noticed all day she was more sleepy she and felt a little warm so she slept most the day so I checked it just now and it’s now 103.4 chills and fever but still smiles and eats and diapers are wet. Just gave her more meds and now she fell asleep I’m hoping for the night ... should I check her in another 4 hours? When should I call her doctor or take her to the ER? She also takes a small dose of antibiotics daily for urinary reflux so she doesn’t get any More kidney infections or UTIs.... she’s 17-18lbs so I gave her 2.5ML of Tylenol that’s all I know to give her
