I have 2 questions and you guys don't have to answer both I just need answers from people with experience.

1. What paperwork do I need to get maternity leave ? (screenshot up there for NY State paperwork), and I'm submitting it prior to the baby being born so I don't quite understand how to do this.

Everything in the blue bar that says *bonding form/certification* I need to know about.

2.what do me and the father need to do prior and after the baby is born? (what paperwork, what to do, etc).

I need to know because my mom hasn't given me anything and I'm scared if I don't have anything ready and my anxiety kills me.
