could I be dialated??

I am currently 38 weeks pregnant and I've been feeling some pinching almost ripping sensation like I'm cut down there in my vag and I'm wondering if I could be dialated?? Reason I ask is because my mid wife hasn't checked me down there at all I go in ever Tuesday and last Tues I'd thought she would check but didnt?! idk I guess every practitioner is different? just wondering if any of you ladies have had the same feeling and what you've been told?
