two sacs?!

Question

I went in for an ultrasound today due to bleeding and spotting.
The technician measured the sac that in circled in purple and asked me if twins run in my family. I then ask if there is only one baby and she says yes.
I am so confused. Does this look like an empty sac?!

Yes, I had in the beginning of my pregnancy a period bleeding (a lot of blood) I thought that I had a miscarriage because I already had a positive test. But after the bleeding (it was only a whole day, normal period and miscarriages are painful and longer) and I didn’t feel a lost (sometimes you read about it and the woman feel hurtful) so I called the midwife and she said that she would do a ultrasound just in case. 
At the ultrasound I saw the same, a little bean and a empty space next to it. Midwife only said that there was a chance that I would maybe bleed again? She never confirmed my feeling/thought that it could be a twin pregnancy and that I lost one. I also am in my own calculations 38 and 4 days. While the baby measured 37 and 4 days. 
I hope you have a good pregnancy and take the time, normally your gut feeling is right, so if you think you had a twin, then I think it was.
