Umbilical cord stump care

My newborns stump seems to always rub on her diaper no matter how far I fold. She's small so the newborn diapers are a bit big on her. Should I switch to preemie size diapers? & Just today a little yellow speck popped up on her stump and I'm afraid it's infected. What do you guys think?
