My boobs are so complicated !

I had a baby a year and 7 months ago..I breast fed from my left boob more due to having a flat nipple on my right..I only breastfed a week or so then stopped. Time went on and I started noticing my right nipple is extremely huge and it literally almost covers my whole boob. My left one is ok and is normal but my right boob is pointing down, huge areola, I have 3 holes on my areola, I have really bad pain in the bottom of my areola like an 1/2 inch under the nipple and it feels like a little lump. It hurts to the touch or even if I rub it..I'm only 22 yrs old and could it be because there's milk in there still or a blocked duct? Idk...I'm currently pregs. And due in feb. I'm scared it's gnna get worse anyone else have holes and bumps and pain and a huge boob that points down because of the weight..shouldn't my left boob sag since I breastfed more with that one? Someone tell me something! I wanna go to doc but I'm afraid they will do mammogram which my mom says it hurts a lot...idk
