Trouble getting organized

Im at a point in my life where im starting to get alot of responsibilities and im facing more and more stress. I want to start getting my life organized i have high school, online college classes, a job, social life and meditated me time. There are days i go straight from school to work then going home at 9pm to work on college and homework leaving almost no time for myself. There just isnt enough hours in the day. I have been told ways to get organized but cant figure out how to lay a plan out. Does anyone know of a outline or good layout i could use. I would love to start drinking more water eat better and start walking around the neighborhood to get more exercise. I live a very lazy life when im not overwhelmed by things that need to get done amd i dont like it anymore. I feel gross im tired all the time and i know im heading down a bad path. I need motivation and a ohanized plan.
