Uncontrollable diabetes

I’m 19 years old and have been a diabetic for 13 years, out of those 11 years I’ve had a ton of control of my diabetes, keeping my A1C at a 6-7 and eating very healthy until the year 2018-2019 in these years I’ve had DKA 5 times and was in the hospital for up too weeks at a time, my A1C is currently between 11-12, I’ve been to my doctor multiple times and I’ve followed all her instructions I’m eating healthy, a lot of fruits and vegetables, meat and protein, good carbs. I test my blood sugar up too 5 times a day and I do insulin every time I eat or I need a shot, I don’t drink soda or juice, I drink a lot of water and tea. My numbers are constantly 400+ tho. Any diabetics out there have any advice or tips it’s greatly needed and please no judgement I am trying my hardest to get control of it again but I also suffer with severe bipolar depression and major social anxiety.
