Where to go for first prenatal visit?

I'm about 4 weeks pregnant for the first time, and don't have an established OB/GYN provider. I want my pregnancy and birth to be as natural as possible, but I haven't even come close to deciding if I want to pursue a home birth, birthing center, or hospital for delivery. I have a few tours set up over the next few weeks, but am wondering if I need to make an appointment for 8-10 weeks quickly. Is it important to have your first appointment at the place you want to give birth, or is switching common? I'm also very undecided about ultrasounds, as I've read worrisome things about their safety, and don't want to be pressured into one. My insurance, however, doesn't currently cover any birthing centers. I will switch before birth to one that does, but until September, only hospitals and clinics will be covered. Should I just schedule with an obgyn or midwife at a clinic, and then take time to explore my options? Or get in with a midwife at a birthing center early and hope they bill everything after the birth?

I saw my primary first and he referred me to my midwife.
No birthing center will bill after birth.  Too many people would skip out on payments otherwise.  I have to have mine paid ($1500) in full by 36 weeks or I cannot deliver there.  First payment of $500 has to be paid by 16 weeks.  It’s tight but doable.  It just takes a bit of saving.  A lot of hospitals have a similar policy.

You can switch providers at any time. I’ve had friends switch at 35 weeks! The only caveat to that is the provider’s availability (depending on the type), and their personal policies regarding new patients. 
My home birth midwife didn’t need to see me until 12 weeks and we’d selected her before I was even pregnant, so if you’re going to go that route you probably have some time. She’s also been incredibly respectful of my informed consent and has been 100% on board with my wish to avoid unnecessary tests and ultrasounds.

Answer

You can be seen by midwives at a birth center but still choose to deliver at a hospital. A midwife will be with you and act as your advocate.
