If I take an antibiotic 4 times daily when should I take it again?

I didn’t get my first dose today until 11am and now it’s 1pm. I wasn’t able to fill my prescription last night, that’s why I was late taking it this morning. I’m just confused on how to take it 4 times daily.

Take 24 and divide by how many times a day you take it. Thats how many hours to take it again. 
24÷4=6, so you take a pill every 6 hours. So 5pm is the next dose. It may sound wrong, but "per day" means "per 24 hours," not by calendar day.
