Could be from an incorrect latch. If it feels "pinchy", the latch is incorrect. Make sure baby has his mouth wide open and your nipple and a large part of areola is inside babys mouth.

Totally normal. Invest in a good nipple cream if you have not yet. My sister-in-law also used these silver cups to place on your nipples between nursing sessions that she swore by. I think you can find them on Amazon?

Could be a clogged duct too - dangle feeding and hot/cold compress helps!
