Usually you can easily tell the difference between a pregnant woman and fat, and in my opinion if you can't tell the difference out of respect you shouldn't say anything.

Pregnant or not, being called fat is offensive. My own biological father called me fat (on my first mother's day) I told him I'm pregnant and there's a difference and he said, "no, you're just fat." It was just another reminder to me of why he's no longer permitted in my life.

Not at all, I guess it's different for me though, I have lived my whole life underweight. I was always under 90lbs and I'm 5'4 I tried all my life to gain weight and I never could, now during pregnancy I've weigh 112lbs and I don't mind if people call me fat, I love it... It's a compliment to me, shows me that I'm actually gaining weight and I'm actually healthy now! I know I'm not fat so but still
