Low Andral Follicle Count, normal AMH Long term Endometriosis Dx

Mimi

Hi,

I had a very puzzling visit with my fertility specialist regarding my fertility situation.

Some context: husband and I are hoping to conceive in the next year. I took out my Mirena in jan 19 to see what my situation is as I’ve been on hormonal suppression for my endometriosis (OCP and/or mirena) since I was 13. I’ve had 6 laparoscopic excisions of endometriosis ranging from stage 3-stage 1 until my specialist said I’d be risking my fertility. Last surgery was in 2016 and since I’ve been managing with diet, yoga, acupuncture, seeing a women’s health naturopath and a TENS machine.

My question is: my fertility workup came back with an andral follicle count of 8 (one on left, 7 on right), which he said is low, but my AMH was 3, which he said was good for my age (29).

My FSH was 7 and LH was 9.

Why would my AFC be low when my AMH indicates normal ovarian reserve? Has anyone had any similar experiences, and has this affected their fertility?

He has recommended starting letrozole and then possibly looking into IVF in approx 10-12 months if this isn’t successful.

Thanks!
