Help please

My 4 month old boy has bronchiolitis, hes had it for a month now. We went in about a week ago and they told me not to worry to much since he is able to get the mucus and stuff out of his airways well enough. They did tell us that it could get worse and he might need to be hospitalized and put on oxygen. I dont know how to tell if it's worse. He hasn't been sleeping at all lately, hes had a low fever for a couple days now. Today he was coughing pretty bad and threw up and his nose keeps getting clogged no matter how much I suck it out. What can I do for him? Does it seem like I should bring him back in? I dont know what to do anymore.
