Disclosing an STD

How do you tell someone that you really love and care for that you have an STD (and I didn’t get it by cheating). We went our separate ways for awhile and we’re both in relationships but we’ve loved each other since high school but I’m scared he’s going to look at me differently. I really need some help because I know it’s going to break his heart. I’m also on medication.
