HELP! Sneak peek address?!

I just finished taking my sneak peek sample and it says that the shipping bag is provided but there was none in my box or in the bag it was shipped in. Does anybody know the address that I’m supposed to send this to? Because online there’s an address to sneak peek and also an address to a Gateway Genomics and I don’t want to send to the wrong place. Does anybody have the test with a prepaid envelope? Could someone give me the address that’s on there? I just want to make sure I send it to the correct place. Now I’m having anxiety because there isn’t a quick and easy way to get a hold of this company and I have no idea where to send my test.
