No fetal heart rate obvious - 7 weeks

I started spotting that turned to brighter red bleeding when wiping for a few hours. Now I’m back to pink. I went to the doctor and had my tests done. Suspected subchorionic hemorrhage next to gestational sac measuring 1.4x2x0.5cm

Gestational sac identified but yolk sac not identified.

Fetal pole crown rump 1.1cm

No fetal heart rate obvious.

How can I have a fetal pole but no yolk sac?

Impression: failed pregnancy possible, early pregnancy not excluded.

My hcg levels are at 59 425
