Miscarriage?

I'm around 7 weeks pregnant, this is my 2nd pregnancy, I'm 36. First pregnancy was all good, except had a couple of hours of light bleeding with no big pain around 12 weeks.

I had my first dr appointment 3 days ago, they took blood test and offered internal ultrasound but I refused as I heard it couldn't be good. Anyway, they ended up taking abdominal ultrasound and said they can't see much when its so small.

Around 4 days ago I started seeing small brown discharge sometimes, and I also remember I was feeling faint in the mornings. I kind of ignored it as i had brownish discharge sometimes with my 1st one too, and it was fine. Looking back, i did not feel as tired any longer these past days.

Then, about 30 hours ago i started bleeding the color of a regular period. I would say i changed about 5 panty liners during this time. Not heavy bleeding but a good amount for a little over a day now. There was no pain in the beginning, however after about 12 hours i started having very sharp constant cramping. It eased off after a couple of hours but i have been having constant dull pain ever since.

I wonder if I am in the middle of miscarriage, or could it be hormones acting up around the time my period would have been due? I'm still hoping it will be ok.
