TMI spotting question with picture

I’m currently 18w6d with twins and I started spotting this morning. Last week I fell but went to Er and every thing was okay. Now I’m spotting and worried. I had to leave a message with the nurse but if this was you would you go to Er? Any feedback is greatly appreciated
