teething or sick?

so my baby is 3 months going on 4 months on the 5th of next month. he’s on nutramigen, he was having yellow seedy poops once to twice a day but for the past 2-3 days i’ve noticed he’s more irritable, having very watery loose stools like as if he has diarrhea and pooping like 3-5 times a day. he seems to have lots of gas and in discomfort, he’s only feeding a little and puking it. he is still having wet diapers. he’s also having a lot of drool and always has his hands in his mouth and he likes to bite on things. should i take my baby to the pediatrician? not sure if he teething or if he is sick. 🥺
