Miscarriage

So I’m pretty sure I had a miscarriage last night. We went and checked on the baby and had an ultrasound yesterday at 4pm and it has a heartbeat of 136. I went into ER with heavy bleeding. They informed me everything was fine. Baby was perfect. We got home and around 7pm my cramps were super painful and I explained to my husband they kinda felt like contractions. Around 11pm they stopped and when I stood up I felt something come out. I’m pretty sure this is a baby but would anyone else let me know what they think please. Also could there be anyway I was having twins and just miscarried one? 6 weeks
