Did you still have hCG in system? How far along we’re you?

If you still have HCG in your system (which is highly likely given your level was 400 just 4 days ago) then the opk is picking up on that and is incorrectly positive. If you were to take a pregnancy test I would bet that would be positive and if it is you aren't on peak fertility you are still in the process of miscarrying.
