I'm 18 weeks pregnant does this look like my mucus plug? My urine is strong because it's the first one in the morning!

I have no idea what that could be! It wasn't in the toilet before you used it, was it?! I'd contact your doctor if you're sure it wasn't there has has come out of you. Good luck! I hope you get some answers.

That doesn’t look good. Contact your OB asap. 
Do you suppositories?? It could very well be that if so. Not as concerning.
If it is your mucus plug, while it typically regenerates, loosing the whole thing in one piece could be a sign you are dilated.
