First dr appointment what to expect?

So I'm 4 weeks 4 days pregnant! We've been trying since September, I was put on clomid in late may early June. I've had 2 missed miscarriages in the past so im alittle nervous. It doesn't feel real right now. I'm kinda numb about it. I'm assuming when I call my dr prob on Monday that she will tell me to go get blood work. When will she want to see me? I'm hoping to get an ultrasound around 6 weeks because I'm going to Maryland to see my cousins when I will 7 weeks and I want to tell them. Also being my 3rd pregnancy, my belly is getting big fast. I'm skinny and I'm wearing baggy shirts but I don't feel like that's going to work much longer. Also I just had a Pap smear done, so what would happen at my first appointment?
