What advice can you give for first dr appointment

I am 4 1/2 weeks along, and I work in radiology. A co worker broke the news to me. Di di I twins. Floored and panicked to say the least. I was so prepared for a singleton pregnancy I feel like I don't even know where to start. What questions would you have asked your midwife for that first visit ?
