Is it a safe bet that I should head to the hospital?

38 weeks. Baby has dropped so much I look like I am not even pregnant. Pressure to the point I feel like the baby is going to pop at any moment. I have had diarrhea so much the last couple days. Contractions are felt but don't hurt due to a very high pain tolerance and are about 5 minutes apart.
