Ive gotten them when I get super sick and my immune system is weakened for as long as I can remember. According to my parents It's my dad's fault for kissing me with one when I was a baby. But you can take Lysine (vitamin to help with lip repair etc) and it helps prevent them (:

I get mine every year around the beginning of Winter/late Fall. I think it's just the cold weather that brings it on for me. Not sure why I had one in the first place, but it's happened since I was 10.

I got my first one from sharing a cup with my friend who had one when I was like 2. I rarely get them anymore, sometimes if I get sunburned really bad. I did get a bad one at the beginning of my pregnancy that took forever to heal.
