Maternity Leave??

My employer gives me 16 weeks of 100% pay of maternity leave. Im a bit confused on how everything works since it's my first baby while employed. My doctor considers my pregnancy high risk and she is thinking of stopping me from work soon. My next appointment is January 30. If she decides to stop me then i will be 29 weeks which means i have 11 weeks left until birth. So would i file for the maternity leave at that point? Because that would only leave me 5 weeks with the baby and that is not enough! Or would i file for disability and once the baby arrives i file for the maternity leave???
