can't orgasm during sex

when a girl says that she cannot orgasm during sex, does she mean that she cannot have a vaginal orgasm through penetration? Or does she mean that she physically cannot have any type of orgasm while having intercourse? Like, is "I can't orgasm during sex" basically the equivalent of saying "I can't have a vaginal orgasm"? Or does it mean that having sex makes you incapable of having a vaginal or clitoral orgasm?

Side note: when I have sex, I don't have a vaginal orgasm, but I can make myself orgasm by rubbing my clit while my boyfriend is in me. Does that mean that I can or cannot orgasm from sex?
