TMI: no picture just tmi subject

I have had constipation really bad and after about 2 days it turns into full blown diarrhea. And it never comes out all in one go and it's terrible and lasts hours. Last night I noticed my genitals swelling and I've never had that before but looked it up and it seemed normal. I'm almost 24 weeks. But what concerned me was as I was feeling down there to confirm how bad swelling was, it felt like my baby's head was right there. And it hurt. It hurt to walk, to sit, and honestly every time I pushed to poop it felt like I could push my baby out. It kept me up all night and I woke up after my doctor had already closed. Should I be terrible concerned?
