Moving with a 9 month old

Shortly after my daughter was born in November, my mom was diagnosed with Glioblastoma, the most aggressive form of brain cancer. I live in my Illinois and she's from Delaware so I have only been able to support her emotionally and mentally through the last nine months of treatment. I've been wanting to move home for a few months now to take care of her. Now it looks like I may be able to get a job in Delaware. And we are planning on moving there in September so that we can take on care of my mom. But I'm worried about moving with my nine-month-old daughter. Does anybody have tips and tricks for moving Long distance? It's at least a 12 hour drive.
