Piercing help..

So ive had my belly button pierced for a month now and it just looks disgusting! Ive had it pierced before and it rejected, I don’t know if it’s rejecting again or if I’m just not taking care of it as well as I should be. I clean it twice a day, morning and night. It does get pulled on and hit every now and then. I noticed that when it’s pushed forward or back, the top hole is ridiculously huge. Any tips on how to help this or what could be happening?
