Relationship Advice?

He has been consistently giving empty promises, we don't go on dates. The other night when I was at his house, I had a serious pain so the paramedics came. He didn't go with me so I sat in the ER in an intense amount of pain, after being boarded into an ambulance, alone for four hours.

I love him so much, but he keeps letting me down. I summarized everything, so more has been going on.

He says he needs me, that he wants to spend my life with me, that he is willing to fight for us and fix our relationship, but I'm wondering if there's anything left to fix.

I don't know why I'm posting this, but here I am telling you about my problems.

I guess I just want to know if I should give him a chance. Let me know what you think.
