Painful Menstrual cramps at 33 weeks

My cramps are so strong they woke me up from my sleep, I’m super nauseous and having low achey back pain. I’m tired of going to my doctors office and then telling me I’m okay. (Been bleeding on and off through out the pregnancy they always tell me I’m fine and I get a bill)

I don’t know if I should be concerned or not.
