in hospital very confused

So my nurse checked my cervix cause I'm spotting at 23 weeks. She says I'm not softened or shortened but my tip has started to dilate. Now there's not been sex or anything else to cause this. What the hell does that mean? She couldn't give me answers. waiting on doc but he's in a c section
