My doctor gave me a one page sheet of questions so I just filled that out

Our hospital encourages it and includes a form with the pre admission packet. I consider it more "birth preferences" than a "plan"

Jotted some things down but I'm not particular about anything. As long as the end result is a healthy baby and mom.

I have one. It’s less than a page long and explains my preferences for things like relatives calling for updates, visitors, feelings about pain management, and my priorities in the event of complications. 
My husband and I are in healthcare and have spent years seeing people when life has gone wrong. We have pretty strong feelings about when to resuscitate and when to let go. We wanted to be sure the staff understood that and spelled that out on the birth plan.

I filled in the birth plan form my hospital of choice sends when you preregister. I keep my requirements and expectations pretty loose as labor rarely goes as planned. It is more to get my preferences in writing just in case.

I made one! However, mine states that "I understand this birth plan may need to be altered and I will fully comply with whatever is deemed medically necessary for the healthy and safety of my baby and myself." Its short. Polite, Non-demanding. Pretty much is a paper outlining my wishes/hopes for labor and delivery. Helps me have a piece of mind as well, because it also outlines my wishes/hopes in case an emergency cesarean is needed or if I am unconcious for any reason. Its by no means something they HAVE to follow to a T, I know thats unrealistic! To me, its a tool that helps me deal with my anxieties and answers a lot of questions they may have.
