furious with gp practice

hi all im after some advice please. im in the uk and had my 12 week (first) midwife appointment today. all was going well until i left the room i was sat in with my husband and midwife to go to the toilet for a urine sample. as soon as i was in the hallway the midwife pulled me into a private room and said my gp had ticked boxes saying the following.. i was abusing drugs and alcohol. i am a victim of domestic voilence. that we are under supervision from child proctevtive services... the list goes on. absolutely none of this is the case at all and is all some kind of referral error. the midwife cant remove it from my official records and said if its a mistake a gp / doctor has to remove it. so i called my gp practice and demanded to speak to the practice manager.. that was 4 hours ago and i havent had a single call back. my husband is devastated that they think these things of him/us. i really dont know what to do next to sort this put. help :(
