Very easily. They can do this as early as 5 weeks to detect an ectopic.

Sometimes. Once in awhile they cant quite tell, which happened to me. I ended up miscarrying on my own, but for two weeks I had an ultrasound every other day to try and determine where the zygote had implanted and they never figured it out.
