TTC appointment tomorrow! Advice please!!

We have been ttc on and off for 3 years (I need a break sometimes due to disappointment and anxiety) so I'm going to an obgyn tomorrow to try to find out what the hell is going on. My cycles go from 30-50 days and don't get positive OPKs when I should or sometimes at all (maybe I'm not testing for long enough?) From what I've read I think letrozole would help. Anyhow ..

What can I expect at my appointment? Do you think they'll run tests and put me on some meds or tests first and meds after that or .. ?
