IUD can cause random bleeds. The likelihood of you being pregnant is less than 1%, only a pregnancy test can tell you.

No idea. I mean if it is a miscarriage typically you can take a pregnancy test and if it turns out positive then you have your answer. To me though it literally just looks like your period. Although you have an IUD and don’t normally get periods - doesn’t mean there can’t be those couple times you bleed and have some sort of period
