Birth Plan Help?

Hi ladies, I’m trying to write my first “birth plan” but I’m not sure how to. Could someone give me some tips or a quick list of the things that need to be on it?

Read birth stories and figure out what you want your ideal birth to look like. Remember to be flexible. Birth plans are the ideal but it's ok if things change. 
Pain meds vs no pain meds and do you want them offered 
Do you reserve the right to eat and drink
Fetal monitoring: do you want it at all? Continuous? Intermittently used? 
I would suggest looking up birth plan formats (you can Google them), reading birth stories, and figuring out what you want your birth to look like.

Birth plans only work if you write them in a way the communicates preference not " my way or the hi way " that being said : include things like medication preference (natural, iv, epidural) skin to skin preference (right away or postpone) who you want to cut the chord and if you'd like that delayed and how long. 
If there's something you want to make absolutely 100% sure happens make sure your OB knows before hand and that all the nurses know and I'd go as far as taping it up on the wall (my hospital has whiteboards ) 
I will be writing in big bold letters on my whiteboard DADDY DOES SKIN TO SKIN UNLESS OTHERWISE NOTED

Mine is looking more like 'a list of my birth preferences' than anything. Basically all it says is i would like to avoid an epidural, i would like to avoid medications unless absolutely necessary, and I would like to avoid a c section unless absolutely necessary. 
I have other preferences, such as skin to skin time right after birth and dont wash the baby right away, but i found that my hospitals policies aligned with what i wanted there, so I wont need to include them.
