I received a sheet of gift cards inside a sample box from Infamil and I used those ones and when one of my orders came in they sent another sheet of them. They need to be used a week from when I activate them which I am about to do tonight, you will need to pay shipping. Price ranges depending on which one you choose. When I ordered I chose the cheapest price which ranged from $11 and some change - $14 and some change. I personally thought that wasn't a bad price. Let me know if you're interested and I will send you a picture of the back. Just so you know, none of these are linked to me in anyway.
