Also depends on the state. I live in a mandated state that requires infertility coverage by employers. Although there are a few exceptions.

I have United Healthcare for insurance and they cover 80% of IVF after you have met your deductible. Check to see if you have insurance coverage!

It’s all very complicated. It depends on your company’s insurance in addition to your state as well as the plan you pick. For example, we choose a high deductible plan because we typically do no use our insurance outside of check ups. We in turn pay much lower premiums. You can choose a no deductible plan and pay what appears to be 0% on things like ivf but you’re paying typically much higher premiums. It often balances out. I have never heard of the over 35 rule. That sounds like a company policy, not state policy. My state does no hAve age restrictions on infertility coverage.

I’ve lived in a state that mandated IVF and my insurance covered it 100% but there are limits. Like they won’t use your eggs if you are over 35. And of course the insurance didn’t cover donor eggs. So I’ve learned IVF coverage does not guarantee you will be helped at all.

I live in Illinois, which is a mandated state but there are caveats to the mandate. My husbands company also covers infertility. My ivf cost me my deductible, which was $3k. Cryopreservation which wasn’t covered by my insurance was another $800.
