I agree with ashley that it might be an obstructive type of an issue but it also might be abnormal production of most of the sperm when it comes to their tail and neck parts. Sperm can show abnormalities in neck and tail which will affect their motility but if the head is fine the morpholigy is good. Some of them just swim in circles. Their engine is just at the beginning of the tail behind the neck it acts as a propeller but it is made of mitohondrial type of dna which is bacterial in origin. If the engine is deffect they cannot swim far fast or straight. It might be also high testestoren or low testesteron issue or all hormones disbalanced. Testosteron is a steroid type if a molecule in high doses it mutates things and overstimulates the testicles so production lowers. Too low and production again mutates and dwindles. Some hormanal check is good idea. Hormonal issues effect more count and motility than morphology in most people. The immune systems white cells might be attacking them too. Varicocelle and any onstruction in vas difference will have similar consequences.

See a urologist to find out the why! There could be blockage for the count issue. If you can get the count up then motility issue can be solved with an iui. 
My husband had low count, morphology and motility were fine. We ended up being successful with ivf (icsi)
