What information to leave with babysitter?

What kind of information or care sheet do you leave with babysitters/caregivers? Looking for all ideas!

Where we’ll be
When we’ll be back
When to feed
How to prepare bottle/food
When to put to sleep
Step by step nightly routine
Where everything is (blankets, paci, swaddles, PJs, etc)
Tips on what to do/things to try if baby won’t fall asleep or if baby is fussy

Everything Mel said, and the address/phone number of where you will be while the babysitter has your children. I would also write down just simple instructions for situations someone who isn’t around your kids often. For instance, my son needs his stuffed puppy to go to bed, and sometimes it’s lost but it always ends up being in the same place (behind the couch) so I would write “Stuffed puppy is probably behind couch, if *name of kid* needs it to go to sleep*

Answer

I left the numbers to reach us (cell phones and hard lines of our location) and basic information she may need in case of emergency - our son’s date of birth, known allergies and conditions, and dr’s name. 
We also left a schedule with notes for her since it can be a lot to remember when we toss information at her the moment she walks in.
