Baby won't go to sleep at night

Every night we have the same problem, it started with colic and gas, that we managed to take under control, now he won't go to sleep by his own. I feed him, burp and then start to rock and walk around the house, then he falls asleep, I put him into the crib and 5min later he's up again, and after awhile he starts to cry hysterically, while his eyes are falling, like he does not want to fall asleep, anyway what to do to help him to go to sleep and stay that way?
