Affidavit of relationship?

Hi mommas, doing my FMLA paperwork and they want a Affidavit of relationship. Do we get the birth certificate of baby in/from the hospital once baby is born? Then send a copy of it afterwards? Sorry FTM.

Answer

I think you get temporary birth certificates from the hospital before you leave. Then you file for the state ones after. I think my work accepted the hospital document.
