Is depends on what you mean by bleed. Spotting is fine, but when you start bleeding red blood and it's more than a 10cent piece then that's what you should get checked and may be a possible miscarriage.

Eh it can depend. Unfortunately at 4 weeks it will be harder to tell because although usually they say if it’s large clots or quickly filling a pad get checked out etc. but because you’re technically around the time of your period (yes you have a positive test and yes it’s still technically a miscarriage-chemical) and those things are worrisome in general. It’s likely if the bleeding is similar or a tad heavier than your period that you’re experiencing a chemical and if you want to get checked out, do so, but likely they will take your blood and then take it again 2 days later to see if it’s rising or falling and tell you to monitor. 
Now spotting it different and it’s totally normal. Keep an eye on any spotting or bleeding and you can report it to your provider. But it seems like a different story 4w VS 12w at 4w the bleeding will likely not progress to the “quick call the doctor” stage because you’re around the time of your period. But at 12w the bleeding could be and usually will be more worrisome in a sense because you’re not just due and the bleeding can and will progress quickly. 
I hope everything is okay. Monitor your bleeding if it progresses I’d let my doctor know but I wouldn’t rush to er as there’s not much they can do as at 4w they won’t see anything on ultrasound and will likely tell you you’re miscarrying even if you’re not because they can’t see anything and there’s no way to know if your beats are rising so they will cause more stress as they will say you’re likely miscarrying but to follow up with your doctor. So I definitely recommend contact with your doctor VS going to the er.

Yes you can bleed in early pregnancy but it wouldn’t hurt to go get checked out because unfortunately you could also be miscarrying
