Those who are pregnant with a “surprise” baby, how did it happen?

I know birth control methods aren’t 100% but I’m interested which methods have “failed” the most. 
And to add: was the method used correctly or was it a mess up

My daughter was conceived with the mirena iud, which is incredibly rare, even more so to have a successful and perfectly healthy pregnancy. I conceived with the iud still in and only found out I was pregnant when she got big enough to push the iud out. I had lots of bleeding but  she was perfectly fine. The doctors called her a miracle baby!

My first it was the pull out method it’s worked for 3.5 years, till it didn’t lol
My second we tried for 8 months
My third we pretty much only had sex with condoms here and there, and also the pull out. Then one night our other two kids were at their grandmas, I checked to see if I was ovulating I wasn’t (or so I thought) I was a few days before my period so we were dumb, he went inside and now here was are 7.5 months later with our 3rd and last lol I got my tubes tied! No more oops. ￼

I wasn’t trying to prevent pregnancy because I was told multiple times I couldn’t have a baby... a week after my last fertility appointment where I was told the same thing again... I conceived naturally after not having a period for 8 months. I didn’t find out I was pregnant till around 10weeks. Good surprise for sure!

Well my method was different, my now  ex boyfriend didn't want me on any kind of birth control as I seem to gain weight on them. So I tracked my cycles with OPKs and I had rather perfect cycles. 
Thanks to the covid 19, my usual brand of OPKs were sold out and I had to switch from digital to the two lines.... the night I saw him (and the night that caused me to conceive) I hardly had a line, like I really had to squint to see it and i was still 4 days from my predicted O date.... the next morning the line was like "BAMMMMM!" Almost darker than my control line. 
Currently almost 6 weeks pregnant.

I had the copper coil which didn’t work! Had to have it removed as an emergency!

Had my IUD removed, several months later i got engaged, had sex around Christmas, then surprise pregnancy. 😂. Pull out method failure

So with my first 2 pregnancies, I had to take clomid. Tried for my first baby for 2 1/2 years, my second for a year. So my 3rd, we thought that I couldn't get pregnant on my own. We used the pullout method (we know, not reliable but like I said we thought chances were slim anyway). With my 4th, I was on the mini pill and I accidentally missed a couple of days. Again, thinking my 3rd baby was probably just a fluke, we thought chances were slim and did not use additional protection. My 4th is due in December. My husband is getting a vasectomy this time. 😊
