Just wondering

My boyfriend and I had protected sex.. he NEVER came in the condom & it was a week after my ovulation date. After he got done, I took the condom off and threw it beside the bed.. later I tried to do the water test and noticed it was a hole near the tip of it.. I’m just wondering did the hole come from me throwing it beside the bed and me stepping on it or if it was there when we were having sex because the way it looked I feel like the hole would’ve been bigger if he would’ve kept going.. & this happened a week after ovulation and he never ejaculated.
